Eligibility for Home Care

To be eligible for home health care services under Medicare and most insurance plans, patients must meet the following requirements:

  • Patient must be homebound
  • Care must be part-time or intermittent
  • Patient must have a need for skilled care
  • Patient's physician must authorize home care services and participate in the plan of care

Referrals

Metrostar Healthcare Services accepts referrals from hospitals, friends, family, social workers, care coordinators, physicians, and yourself, the patient. Download our referral form.
